NOTE: The postal cover was signed contemporaneously with the launch, however the disaster coin and “25th Mission Disaster” designation were clearly added at a later date. However, a structural failure during its ascent phase 73 seconds after launch on January 28 from Kennedy Space Center Launch Complex 39 killed all seven crew members.
